A deep, spontaneous sigh often signals relief, exhaustion, or acute tension. Now, innovative wearable technology is revealing that the way we sigh in daily life holds critical clues about our mental health. According to a new study published in the journal JMIR Formative Research, researchers using sensor-packed smart garments discovered that people diagnosed with anxiety disorders exhibit an altered sighing response during moments of distress compared to healthy individuals.
The Body’s Natural Reset Button
Physiologically, sighing serves as an essential respiratory reset mechanism. Taking a breath roughly twice as deep as normal helps re-inflate tiny air sacs in the lungs and regulates the exchange of oxygen and carbon dioxide, while also acting as a physical release valve for emotional strain. While laboratory studies have long linked sighing to emotional regulation, artificial clinic environments lack ecological validity. Researchers previously lacked evidence showing how sighing dynamics unfold in free-living conditions, leaving it unclear whether sighing reflects fleeting emotional spikes or chronic psychological pathology.
High-Tech Monitoring on the Move
To capture breathing patterns in real-world settings, the research team monitored 38 adults—comprising 15 individuals with diagnosed anxiety disorders and 23 healthy controls. Participants wore Hexoskin smart shirts equipped with continuous respiratory inductance plethysmography across four 36-hour monitoring blocks scheduled over one to two weeks. The shirts recorded breathing mechanics continuously, defining a sigh as any breath with a tidal volume at least double the participant’s median level.
Throughout the daytime hours, participants completed six randomly timed smartphone check-ins reporting their momentary anxiety levels using ecological momentary assessment. By coupling each survey response to the preceding five-minute respiratory window, the authors analyzed 1,279 synchronized psychophysiological data windows across 33 participants who had sufficient data.
A Blunted Stress Response
The findings revealed a marked divergence in how healthy individuals and those with anxiety disorders regulate stress. In healthy participants, brief surges in momentary anxiety naturally triggered an increase in sighing frequency, illustrating a healthy, flexible regulatory reflex. In contrast, participants with anxiety disorders exhibited a blunted response; their sighing did not ramp up dynamically in response to momentary anxiety spikes, despite their group having a higher baseline sigh probability overall.
Furthermore, the researchers found that higher chronic, trait anxiety across participants was linked to a lower overall proportion of sighs. This distinction highlights a clear dissociation between enduring psychiatric traits and state-dependent emotional fluctuations in daily life.
Paving the Way for Smart Mental Health Wearables
Beyond shedding light on breathing mechanics, the study proved that combining wearable respiratory sensors with real-time symptom tracking is both practical and reliable. Participants achieved a 79.5% completion rate on mobile prompts, and the shirts produced high-quality respiratory data more than 84% of the time.
By demonstrating that altered sighing patterns can serve as a candidate digital biomarker, the authors believe the research opens doors for smart mental health devices. In the future, continuous wearable monitoring could detect dysregulated breathing patterns in real time, prompting timely and personalized breathing exercises before rising distress escalates into an acute anxiety episode.
